Metabolic perturbations in the diabetic heart: Mechanisms and molecular targets

Abstract Diabetes is associated with altered myocardial substrate utilization,
specifically increased fatty acid (FA) utilization and decreased glucose
utilization. This switch in substrate utilization is associated with decreased
cardiac efficiency and decreased energetic reserves, and can increase the risk
of heart failure. Potential mechanisms include increased delivery of FAs to the
heart, upregulation of FA utilization pathways
via activation of transcription factors such as PPAR-a, myocardial insulin
resistance nd mitochondrial dysfunction. This review will summarize our current
understanding of the pathophysiology of metabolic disturbances in the diabetic
heart and will discuss
potential therapeutic targets.
